New Patient Appointment
New patients should arrive 15 minutes early for their appointment in order to complete paperwork. In order to simplify your initial visit please download and complete a NEW PATIENT FORM. You may also read our HIPPA Policy (coming soon).
If a patient is more than ten minutes late for an appointment the individual may be asked to reschedule. It is the patient's responsibility to bring all records and proper information to maximize clinical care. If proper information is not brought to the visit, this may delay the clinic appointment, diagnosis, treatment, or create duplicate testing.
